Assumption of Risk

I understand that dance instruction and related physical activity involve inherent risks, including but not limited to muscle strains, sprains, falls, collisions, and other injuries. I voluntarily allow my child(ren) listed above to participate in classes, rehearsals, events, and activities at The Facility Dance Studio, and I accept and assume all risks associated with that participation.

Release of Liability

In consideration of my child(ren)'s participation, I release and hold harmless The Facility Dance Studio LLC, its owners, instructors, staff, and volunteers from any and all claims, demands, or causes of action arising out of ordinary negligence in connection with participation in studio activities, to the fullest extent permitted by Arizona law. This release does not apply to gross negligence or willful misconduct.

Medical Authorization

In the event of an injury or medical emergency, I authorize studio staff to secure emergency medical treatment for my child(ren) if I cannot be reached, and I accept financial responsibility for such treatment. I agree to keep the studio informed of any medical conditions, allergies, or medications relevant to my child(ren)'s safe participation.

Electronic Signature

By typing my full legal name below, I confirm that I am the parent or legal guardian of the dancer(s) listed on this registration, that I have read and understood this waiver in its entirety, and that my typed name constitutes my legal electronic signature with the same force as a handwritten signature.
